Player Screening Process




All participants will be required to complete the contact tracing
online screening process below prior to each scheduled soccer activity.

For all players aged 16 years old and younger, the online form
must be completed by their parent or guardian. This process
must be completed and submitted at least 1 hour prior to the
start of the on-field soccer activity.

We highly recommend this process be completed on the same day as the soccer activity to ensure we collect the most accurate details.

Staff, coaches & participants will have to complete the symptom checklist before EVERY on-field soccer activity. Players will not be permitted to train if they DO NOT complete this online process.
